Dental implants

An implant is a screw, an abutment and a crown. That is all.

Most people arrive without knowing what is actually going to be put in their mouth. So we start there: we show you the part, where it goes and what happens at each visit. Once you understand the treatment, the fear drops on its own.

  • The screw replaces the root. It is titanium and goes into the bone under local anaesthetic. Most patients describe it as less uncomfortable than having a tooth pulled.
  • The bone grips onto it. This is the waiting period, usually three to six months. Throughout it you wear a temporary tooth — you will not be left with a gap.
  • The crown goes on last. It is made to match the colour of your teeth. It screws or cements onto the abutment, and that is the end of the treatment.
  • You clean it like a tooth. Brush, floss or interdental brush, plus your check-up. It does not come out at night and needs no adhesive.

About Medicare, with numbers

Original Medicare (Parts A and B) does not cover implants or dentures. It has been excluded by statute since 1965 and that is not changing this year.

Many Medicare Advantage plans do carry some dental coverage, but almost always with an annual cap. The average cap is around $1,300 a year, and 59% of plans with extensive coverage cap at $1,000 or less. Against a $15,000 arch, a $1,300 cap covers roughly 9%.

What we get asked most on the phone

Does getting an implant hurt?

Placement is done under local anaesthetic: during the procedure you should feel no pain, only pressure and the sound of the instruments. Afterwards there is soreness. It is usually worst about six hours later, swelling peaks around two days, and by the third day most people manage on over-the-counter painkillers.

Studies comparing the two find that placing an implant is less uncomfortable than pulling a tooth. We work with local anaesthetic, applied calmly and given the time it needs to take effect. If you think you could not handle that, say so at the assessment and we will talk it through straight. We are not going to tell you it is painless, because it is not.

Does Medicare cover dental implants?

Original Medicare (Parts A and B) does not generally cover routine dental treatment or implants. Some Medicare Advantage plans do include dental coverage, usually with an annual limit.

Bring your card to the first visit and we verify it in front of you. If your plan does not cover it, we work out the monthly payment with financing before you decide anything.

I have been told I do not have enough bone. Is that the end of it?

Almost always it can still be done, even if the road is longer. When bone is missing there are options such as bone grafting or a sinus lift, and in some cases a different type of implant or a different technique.

That is exactly what the 3D scan at the first visit assesses. If in your particular case operating is not advisable, we will say so just as plainly.

How long does the whole treatment take?

From start to finish it is usually three to six months, because the bone has to integrate with the implant. If a graft is needed it can take longer.

Throughout the wait you wear a temporary tooth: at no point will you be without teeth.

I have diabetes / I take medication. Is that a problem?

Not necessarily, but it has to be assessed. Well-controlled diabetes does not rule out treatment; some osteoporosis medications and certain medical histories do call for precautions or change the plan.

That is why at the first visit we ask about your medical history and your medication. Bring the list, even if it is handwritten.

Can I just come for information, with no commitment?

Yes, and it is what we recommend. The first visit is an assessment: we explain the options and you take the plan home. Nobody will ask you to sign or pay anything that day.
